The "Third eye" is the effect you get by eating Demon's Dream Essence, Demon's Dream Fruit, Demon's Dream Seeds, or Otherworld Essence. When the "Third Eye" is applied your screen will turn greyscale with a chromatic aberration and blurry, with this effect you will be able to see and interact with otherworldly leaves, logs, stones, and ores. This is how you get started in the mod. But you only have a 33% chance of getting this affect by eating the fruit. The other 67% you only get a hunger effect. The four listed foods above are obtainable in survival and can get you the effect.
